Emergency Tooth Extraction: When a Tooth Can’t Be Saved

Dentists do everything they can to save a tooth. But when severe infection, a deep crack, or advanced decay has destroyed too much of the structure, emergency tooth extraction becomes the safest path forward. When does a tooth need to be extracted immediately?

Not every toothache is an emergency, but certain situations call for same-day action. A tooth typically needs to be pulled when:

  • Infection has spread to the surrounding bone or tissue and antibiotics alone cannot control it
  • Decay has destroyed so much of the tooth that a crown or root canal is no longer possible
  • A crack extends below the gum line, making restoration impossible
  • The tooth is causing swelling, a fever, or difficulty swallowing, which signals the infection is spreading

That last point matters: swelling around the jaw, a high fever, or trouble swallowing are signs the infection has moved beyond the tooth itself. Get care the same day. This is not something to wait out over a weekend.

When removal is the right call, it protects the rest of your mouth. Leaving an infected or broken tooth in place puts neighboring teeth and bone at risk.

Does an emergency extraction hurt?

The extraction itself should not hurt. Before we start, we numb the area completely with local anesthetic. You may feel pressure as the tooth is loosened from its socket, but that is different from pain. Most patients are surprised by how manageable the procedure is.

How much does an emergency tooth extraction cost?

Cost depends on the complexity of the extraction. A simple extraction, where the tooth is fully visible and comes out in one piece, typically runs between $150 and $300. A surgical extraction, needed when the tooth is broken at the gum line, impacted, or requires sectioning, generally ranges from $250 to $600 or more.

What should I eat after an extraction?

For the first 24 to 48 hours, stick to soft foods that require no chewing. Yogurt, mashed potatoes, scrambled eggs, smoothies (no straw), and applesauce are all good choices. Avoid anything hot, crunchy, or that requires suction.

How do I replace an extracted tooth?

Leaving a gap where a tooth was removed does more harm than most people realize. Over months and years, neighboring teeth shift toward the empty space, the underlying bone begins to shrink, and biting and chewing become harder. Replacing the tooth protects your long-term oral health, not just your smile.

Two options work well for most patients:

Fixed bridge. A bridge is anchored to the teeth on either side of the gap and fills the space with a prosthetic tooth. It’s a strong option when an implant isn’t feasible, or when you need a faster timeline.
